Fall Risk: The 4-Point Checklist

Three questions, a 12-question checklist (4 points or more) and a 12-second TUG: a fall-risk check you can do at home.

Fall Risk: The 4-Point Checklist

A fall-risk check you can do at home in three steps:

  • Ask three questions: a fall in the past year, feeling unsteady, worrying about falling. Yes to any one means you're at risk (CDC STEADI).
  • Score the 12-question checklist: 4 points or more means you may be at risk.
  • Time a Timed Up and Go: 12 s or more means you're at risk.
